Livingston Pediatrics

Patients have the right to the following:

Quality care, delivered in a consistent nature with competence and compassion.
bullet
Services delivered in a timely manner that are appropriate to the patient's needs.
bullet
Confidentiality in regard to medical and personal history, and individual medical records and information.
bullet
To be treated with dignity, respect and consideration.
bullet
To be informed about personal health as it concerns medical conditions, diagnostic tests and treatment plans.
bullet
To a second opinion.
bullet
To involvement in decision-making concerning treatment. 
bullet
To refuse participation in research. 
bullet
To approve or refuse the release of information except when the release is required by law. 
bullet
To create Advance Directives which let providers and others know the person's wishes concerning medical treatment. 
bullet
To assert complaints and grievances about the providers and the health care provided. 

 

Patients are responsible for the following:

To become informed about their insurance plan, including benefits available and necessary co-payments.
bullet
To become knowledgeable of the system to access medical care. 
bullet
To keep all scheduled appointments and to notify the provider in advance when unable to keep an appointment. 
bullet
To be on time for all appointments. 
bullet
To follow all medically appropriate physician orders and prescriptions. 
bullet
To treat all personnel with courtesy and respect. 
bullet
To provide complete health status information for accurate diagnosis and treatment. 
bullet
To always call your provider before going to an urgent care facility. 
bullet
When possible, to contact your provider before receiving emergency care. Otherwise, to notify your provider within 24-48 hours after receiving emergency care, or as soon as possible.
